As a participant in the Majlis Tindakan Pengguna Negara program, I hereby authorize Majlis Tindakan Pengguna Negara to use my name, company/organisation name, likeness of my voice and photographs, including video or other media, without compensation to me or to my company/organisation if I or my company/organisation is selected as an award recipient.

I/We understand that if any of the information provided in the submission is found to be untrue, misleading or is a cause for misrepresentation, the National Consumer Action Council (MTPN) and Organizing Committee of Malaysia Influential Women Entrepreneurs reserves the right to withdraw the Recognition at any point of time may cause to publish such information in any media. Upon withdrawal of the Award, the trophy, the certificates and any other winner items shall be returned to the National Consumer Action Council (MTPN).

CTOS CONSENT
You may conduct credit/trade check including CCRIS checks on me/us and when consent has been given individually, on our directors, shareholders, guarantors, etc. with CTOS at any time for as long as I/we have a trade relationship with you or where any dues remain unpaid and outstanding with you, for any one or more of the following purposes:

√ Opening of account
√ Debt recovery
√ Credit/Account review
√ Legal documentation consequent to a contract or facility granted by you.
√ Credit/Account monitoring
√ Credit/Account evaluation

Apart from the above, I/we the undersigned do give my/our consent to you and the CTOS, to process my/our personal data as per the PDPA Act.
